What caregiving program is offered through medicade in NEW York?

I work as an attendant in Virginia for a child with autism and downs syndrome. I'm paid by medicaid, through a program called public partnerships. My grandparents live with my aunt, and shes barely around, so they have to take care of my cousin, james, most of the time. he has epilepsy and cerebral palsy, so it takes quite a toll on them. Unfortunately, public partnerships doesn't cover the state of new york, but does anyone know what company does?
